2008-09 Incarnation School Registration

New Student Applications are available online, at the school office (weekdays, 8 A.M. to 3 P.M.) or by mail, (call 818-241-2269).

THE FOLLOWING INFORMATION IS ALSO REQUIRED:
For Kindergarten:

• Certified (original) & copy of Birth Certificate and Baptismal certificates.
• "Authorization of Release of Student Information" form. (Complete address with zip code is necessary! Incarnation will forward to appropriate school.)

For Grades 1 - 8:

• Baptismal Certificate
• First Communion Certificate
• Latest Report Card
• Latest Standardized Test Scores (CTBS, ITBS)

ADDITIONAL INFORMATION:
Kindergarteners MUST be 5 years old by 9/1/08. Incomplete applications will NOT be accepted! Non-refundable Application fee of $50 per child MUST be included with application paperwork when returned to office; Check payable to Incarnation School.

PLACEMENT TESTING FOR APPLICANTS:
Kindergarten, February 20-22, in the afternoons. Appointment times will be given at 15 min. intervals. Grades 1-8, Saturday, February 23, 2007, 9 A.M. Grades 1 - 3 will end between 10:30 & 11 A.M. Grades 4 - 8 will end between 11 & 11:30 A.M.
